There are numerous tools developed to assist in the assignment of medical codes (ICD-9-CM, CPT® and HCPCS Level II) from clinical documentation. These vary from basic code lookup software to sophisticated applications that take advantage of Electronic Health or Medical Records (EHR/EMR) and incorporate rules-based encoders and automated coding.
