Get Ready: An ICD-10 Friendly CMS Claim Form Is Here

The National Uniform Claim Committee (NUCC) has recently revised the Centers for Medicare & Medicaid Services’ (CMS) 1500 claim form (now version 02/12) to prepare for ICD-10-CM diagnosis codes. The new code set is scheduled for implementation on Oct. 1, 2014. CMS-1500 is the required form for submitting paper claims to Medicare, so be sure you use up your current forms to stay compliant, and make room for the new ICD-10-CM friendly one.
The CMS-1500 paper form is used by physicians who meet exceptions to electronic reporting. In the new (version 02/12) form, you can indicate whether an ICD-9-CM or ICD-10-CM code is being used. The number of possible diagnosis codes on the form has been expanded from four to 12, as well.
The new claim form is tentatively scheduled to be accepted by Medicare in January 2014; the old forms will still be accepted until April 1, 2014. Check with your clearinghouse to determine for sure when the new 1500 form can be used.
